1. Introduction

Hallmark Precision Health is committed to providing premium, highly personalized medical care while protecting the privacy and confidentiality of your personal and health information. This Privacy Policy explains how we collect, use, store, and disclose your information in compliance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs).

2. What Information Do We Collect?

To provide our regenerative and longevity protocols, we must collect personal and sensitive health information. This includes:

  • Personal Details: Name, date of birth, address, and contact details.

  • Health Information: Medical history, pathology results, specialist letters, imaging reports, medications, allergies, and lifestyle risk factors.

  • Administrative Data: Medicare number (where applicable for identification), billing details, and appointment history.

  • Digital Information: Information collected securely via our website forms, patient portal, and interactions with our digital library.

3. How We Collect Your Information

We collect information only by lawful and fair means. This occurs:

  • Directly from you via our website registration forms, telephone, or during your Discovery Consultation.

  • From third parties (with your consent or where legally authorized), such as referring specialists, diagnostic imaging centers, pathology labs, or the My Health Record system.

4. Why We Collect Your Information

We collect, use, and store your information primarily to safely and effectively engineer your personalized healthspan strategy. This includes:

  • Conducting comprehensive clinical audits and strategy sessions.

  • Managing medical records and clinical administration.

  • Processing secure payments via our gateway partners (e.g., Tyro).

5. How We Use and Disclose Your Information

We treat your personal information as strictly private and confidential. We will not use your information for marketing without your express consent. We may share your information:

  • With other healthcare providers (e.g., specialists) involved in your care.

  • With strictly vetted third-party service providers (such as Zedmed clinical software and Tyro payment systems) who are also bound by the APPs.

  • When required or authorized by law (e.g., court subpoenas) or to prevent a serious threat to a patient's life or public safety.

6. Data Security and Storage

Your health data is highly sensitive. Hallmark Precision Health implements robust technical and organizational safeguards to protect your data from unauthorized access, modification, or disclosure. This includes securing our premises, utilizing encrypted databases, applying role-based access restrictions, and requiring staff confidentiality agreements.

7. Anonymity

Under the Privacy Act, you have the right to deal with us anonymously or under a pseudonym. However, due to the highly clinical nature of our regenerative protocols, it is generally impracticable for us to provide medical services without identifying you.

8. Accessing and Correcting Your Information

You have the right to request access to, and correction of, your medical records. We ask that you submit these requests in writing. We will respond within a reasonable timeframe (usually 30 days).

9. Complaints
